"Faster, Higher, Stronger" book showed a study where athletes were asked what they would give up for an Olympic gold medal and from what I remember a good portion would give up their lives in 5 years for that (or some other crazy time), the pressure and willing to win for professional and aspiring professional athletes is incredible.

Not in professional sports, see http://www.amazon.com/Faster-Higher-Stronger-Generation-Supe... or http://www.amazon.com/Sports-Gene-Extraordinary-Athletic-Per... (similar ideas in both books)

As "Faster, Higher, Stronger..." author explains, doping wouldn't level the field for everybody since drugs affect people very differently. The concept of "fairness" is arbitrary, he mentions the use of altitude tents for example (banned only in one country, it favours richer countries), but somewhere we need to draw the line.

I was surprised at the book promoted at the end. I was positive this was going to a revised excerpt from Faster, Higher, Stronger by Mark McClucsky.

I felt the same way. The sport optimizations I felt like we're from Faster, Higher, Stronger. The hot dogs I read about in the chapter on outsiders and innovation, maybe in a Freakonomics book. I know I read the hospital and checklists before too.
